I (hereinafter referred to as the «Subject»), hereby provide my Consent for the processing of personal data (hereinafter referred to as the «Consent») under the following conditions.

The Subject consents to the Operator processing their personal data in accordance with the following terms:

• Surname, first name, patronymic, gender;
• Date of birth;
• Email address;
• Phone number;
• Other information processed by the Operator: technical device data — IP address, operating system type, browser type, geographic location, internet service provider; information about the use of the Website.

2. Processing of personal data by the Operator includes:

Any action (operation) or set of actions (operations) involving personal data, including collection, recording, systematization, accumulation, storage, clarification (update, modification), extraction, use, transmission, anonymization, blocking, deletion, and destruction of personal data.

3. The Subject consents to the processing of personal data for the following purposes:

• Identification of the Subject;
• Interaction with the Subject, including sending notifications, requests, and information related to the Operator’s activities, as well as processing requests and applications from the Subject;
• Sending promotional materials and information about special offers to the Subject;
• Ensuring the quality operation of the Operator’s website;
• Preparing documents confirming payment for services by the Subject, and other documents related to the Subject’s activities on the Operator’s website;
• Facilitating interaction between the Subject and the Operator for the purpose of providing services;
• Compliance with regulatory and non-regulatory legal acts, as well as decisions, assignments, and requests from government authorities and persons acting on behalf of such authorities;
• Analyzing the interests of the Subject, conducting surveys;
• Ensuring the necessary level of security;

4. Rights of the Subject of Personal Data:

The Subject has the right to request the Operator to clarify their personal data, block, or destroy it if the personal data is incomplete, outdated, or inaccurate.

5. Transfer of Personal Data to Third Parties:

The Subject consents to the transfer of their personal data by the Operator to third parties for the provision of services requested by the Subject on the Operator’s website, or for similar fulfillment of an agreement between the Subject and the Operator, as well as for audit checks.

6. Data Processing Period:

Personal data of the Subject will be processed until the liquidation of the Operator.

7. Data Protection:

The Operator takes all necessary and sufficient organizational and technical measures to protect the personal information of the Subject from unlawful or accidental access, destruction, alteration, blocking, copying, dissemination, and other unauthorized actions by third parties.

8. Withdrawal of Consent:

Consent may be withdrawn by the Subject or their representative by submitting a written statement. Consent can be revoked provided that the Operator is notified at least 30 days before the intended date of cessation of data processing.

9. Further Data Processing Upon Revocation:

In the event that the Subject or their representative revokes Consent for the processing of personal data, the Operator reserves the right to continue processing personal data without the consent of the Subject if there are grounds specified in paragraphs 2-11 of part 1 of Article 6, and part 2 of Article 10 of Federal Law No. 152-FZ «On Personal Data» dated July 27, 2006.

10. Term of Consent:

This Consent is valid from the moment it is granted and during the period of consideration of the User’s requests or until the moment of revocation of Consent.
